Abstract

Continuity of care dalam pelayanan kebidanan merupakan layanan melalui model pelayanan berkelanjutan pada wanita sepanjang masa kehamilan, kelahiran serta masa postpartum. Pada umumnya, proses kehamilan yang dilalui dengan tanpa komplikasi akan menghasilkan kelahiran bayi yang sehat dan cukup bulan melalui jalan lahir, oleh karena itu untuk mencapai kehamilan yang berkualitas harus didukung dengan adanya pelayanan antenatal care yang berkualitas sesuai kebutuhan klien. (Kementrian Kesehatan Republik Indonesia, 2016). Berdasarkan profil kesehatan Kabupaten Malang, angka kematian ibu di kabupaten Malang pada tahun 2015 yaitu 38,11 per 100.000 kelahiran hidup, sedangkan pada tahun 2016 AKI yang terjadi di Kabupaten Malang mencapai 52,78 per 100.000 KH dengan rincian 21 ibu meninggal dunia. Asuhan Kebidanan CoC ini dilakukan dengan tujuan agar mahasiswa mampu menerapkan asuhan kebidanan yang komprehensif pada ibu hamil, ibu bersalin, bayi baru lahir, ibu nifas dan keluarga berencana (KB), sesuai dengan standar asuhan dengan menggunakan pendekatan manajemen kebidanan. Asuhan Kebidanan yang dilakukan pada Ny. ―V‖ didapatkan bahwa Ny. ―V‖ mengalami ketidaknyamanan saat kehamilan trimester III yaitu nyeri pada pinggang dan saat masa nifas yaitu terasa bendungan ASI pada payudara. Upaya yang dilakukan untuk mengatasi ketidaknyamanan tersebut yaitu dengan memberikan inovasi berupa prenatal yoga untuk mengurangi nyeri pada pinggang, dan massage effleurage untuk membantu melancarkan ASI dan mengatasi bendungan pada ASI. Hasil dari asuhan yang telah diberikan dari kehamilan trimester III, persalinan, bayi baru lahir, nifas, neonatus, dan kontrasepsi telah sesuai dengan rencana dan kebutuhan pasien. Diharapkan pelayanan kesehatan terus mempertahankan asuhan yang diberikan secara continuity of care sehingga meningkatkan derajat kesehatan ibu dan anak. ABSTRACTS Continuity of care in midwifery services is a service through a continuous service model for women throughout pregnancy, birth and the postpartum period. In general, the process of pregnancy that is passed with complications will result in the birth of a healthy baby and months through the birth canal, therefore to achieve a quality pregnancy must be supported by quality antenatal care services according to the client's needs. (Ministry of Health of the Republic of Indonesia, 2016). Based on the health profile of Malang Regency, the maternal mortality rate in Malang Regency in 2015 was 38.11 per 100,000 live births, while in 2016 the MMR in Malang Regency reached 52.78 per 100,000 KH with details of 21 mothers dying. CoC Midwifery Care is carried out with the aim that students are able to apply comprehensive midwifery to pregnant women, maternity mothers, newborns, postpartum mothers and family planning (KB), in accordance with standards of care using a midwifery management approach. Midwifery care performed on Mrs. "V" found that Mrs. "V" experienced during the third trimester of pregnancy, namely at the waist and during the puerperium period, when the breast milk dam was felt. Efforts are being made to overcome this problem, namely by providing innovations in the form of prenatal yoga to reduce back pain, and massage effleurage to help support breastfeeding and overcome dams in breast milk. The results of the care that has been given from the third trimester of pregnancy, childbirth, newborns, postpartum, neonates, and contraception are in accordance with the patient's plans and needs. It is hoped that health services will continue to be maintained, and the care provided on an ongoing basis will improve the health status of mothers and children.
