Abstract

Continuity of care (CoC) adalah pelayanan yang dicapai ketika terjalin hubungan yang terus-menerus antara seorang wanita dan bidan. Asuhan yang berkelanjutan berkaitan dengan kualitas pelayanan dari waktu kewaktu yang membutuhkan hubungan terus menerus antara pasien dengan tenaga profesional kesehatan. Layanan kebidanan harus disediakan mulai prakonsepsi, awal kehamilan, selama semua trimester, kelahiran dan melahirkan sampai enam mingggu pertama postpartum (Evi Pratami, 2014). Bidan diharuskan memberikan pelayanan kebidanan yang kontinu (Continuity of Care) mulai dari ANC, INC, Asuhan BBL, Asuhan Postpartum, Asuhan Neonatus, dan Pelayanan KB yang berkualitas (Diana, 2017).Berdasarkan profil kesehatan Kabupaten Malang, angka kematian ibu di kabupaten Malang pada tahun 2015 yaitu 38,11 per 100.000 kelahiran hidup, sedangkan pada tahun 2016 AKI yang terjadi di Kabupaten Malang mencapai 52,78 per 100.000 KH dengan rincian 21 ibu meninggal dunia. Asuhan Kebidanan CoC ini dilakukan dengan tujuan agar mahasiswa mampu menerapkan asuhan kebidanan yang komprehensif pada ibu hamil, ibu bersalin, bayi baru lahir, ibu nifas dan keluarga berencana (KB), sesuai dengan standar asuhan dengan menggunakan pendekatan manajemen kebidanan. Asuhan Kebidanan yang dilakukan pada Ny. N didapatkan bahwa Ny.N mengalami ketidaknyamanan saat kehamilan trimester III yaitu nyeri pada punggung dan nyeri odema kedua tungkai kaki pada kehamilan, saat masa nifas yaitu terasa ASI kurang Lancar. Upaya yang dilakukan untuk mengatasi ketidaknyamanan tersebut yaitu dengan memberikan inovasi berupa senam hamil dan kompes jahe hangat untuk mengurangi nyeri pada punggung,rendaman air hangat kencur untuk nyeri pada odem tungkai kedua kaki Ny.N ,Pemberian jus daun pepaya untuk Kurang Lancarnya ASI Ny.N. Hasil dari asuhan yang telah diberikan dari kehamilan trimester III, persalinan, bayi baru lahir, nifas, neonatus, dan kontrasepsi telah sesuai dengan rencana dan kebutuhan pasien. Diharapkan pelayanan kesehatan terus mempertahankan asuhan yang diberikan secara continuity of care sehingga meningkatkan derajat kesehatan ibu dan anak. ABSTRACTS Continuity of Care is a service that is achieved when there is a continuous relationship between a woman and a midwife. Continuous care related to health professionals, midwifery services are carried out starting from preconception, early pregnancy, during all trimesters, birth and delivery until the first 6 weeks postpartum (Evi Pratami, 2014). Midwives are required to provide continuous midwifery services (Continuity of Care) starting from ANC, INC, BBL care, Postpartum care, Neonatal care, and quality family planning services (Diana, 2017). in 2015 which was 38.11 per 100,000 live births, while in 2016 the MMR in Malang Regency reached 52.78 per 100,000 KH with details of 21 mothers died. CoC Midwifery Care is carried out with the aim that students are able to apply comprehensive midwifery care to pregnant women, maternity mothers, newborns, postpartum mothers and family planning (KB), in accordance with standards of care using a midwifery management approach. Midwifery care performed on Mrs. N it was found that Mrs. N experienced discomfort during the third trimester of pregnancy, namely pain in the back and pain of edema of both legs during pregnancy, during the puerperium, namely breast milk was not smooth. Efforts were made to overcome this discomfort, namely by providing innovations in the form of pregnancy exercises and warm ginger compresses to reduce back pain, soaking kencur in warm water for pain in the odem of both legs Mrs.The results of the care that has been provided from the third trimester of pregnancy, childbirth, newborns, postpartum, neonates, and contraception have been in accordance with the patient's plans and needs. It is hoped that health services will continue to maintain the care provided with continuity of care so as to improve the health status of mothers and children.
